TO ENTER: Answer each question on the survey and submit the completed survey by clicking on "submit." After you submit the completed survey, you will automatically be entered in the sweepstakes. You must answer every question on the survey to be eligible to win a prize in the sweepstakes. You may also enter via mail by mailing in a completed survey form. To do so, send a letter or postcard requesting the CPS survey to PNRC, 3001 S. Hardin Blvd. PBM 201, Suite 110, McKinney, TX 75070. To be eligible to win a prize in the sweepstakes, your submitted survey form must be postmarked by December 15, 2002 and received by December 31, 2002. Sponsor is not responsible for mail in entries that are lost, misdirected or late. All entries become property of Sponsor.
LIMITATIONS: Each entry must be received by December 31, 2002 . Only one entry per person, however individuals who choose to take the Re-test of the Partial Survey by entering their Bonus LoginID may receive ten (10) additional sweepstakes entries. Open only to legal residents of the U.S. and Canada who are 21 years of age or older. Employees of Sponsor, its respective parents, subsidiaries and agencies (the "Entities"), and members of their immediate families, are ineligible. Sponsor and the Entities are not responsible for late or incomplete entries. Void where prohibited or restricted by law and subject to all federal, state and local laws and regulations.
PRIZE: Ten (10) prizes will be awarded. All prizes consist of vacation vouchers provided by Paradise Adventures. The vacation vouchers cover hotel/motel accommodations for two adults over 21 years of age for 4 days and 3 nights. Children or additional people may accompany the user for nominal charges. Additional nights may be available at discounted rates. The vouchers may not be combined with any other offer or promotion. Tips, room taxes, transportation and any other taxes or personal expenses are not included. All rooms are subject to availability. Reservations must be made through Paradise Adventures according to the terms and conditions of its vacation vouchers. Hotels/motels included with the vouchers include Holiday Inn, Wynfield, Fairfield Inn, Best Western or similar properties. Some dining and entertainment discount coupons may be included in some locations. Certain restrictions may apply in different locations. The vacation vouchers and dining and entertainment coupons are valid for one year from the date of issue. Prizes are not redeemable for cash or transferable; and no substitutions are allowed except, at Sponsor’s sole discretion, prizes of equal or greater value may be substituted. All prizes will be awarded.
