You print out the forms, fill them out, give them the receipt, give them the UPCs, and send it away! The only problem I could think of is that you only have one packaging slip, so if they wanted the packaging slip in the rebate, then you might have a problem. But I am pretty sure you could just print out your receipt from the confirmation e-mail they send you.
