Am I correct that upgrading the B drive on an xtreme'd s1 DTiVo can be done by purging all stored programs then running TiVoMAD on the A and new B drives?

What is it you are trying to do, just replace the B drive? You can't do that without first creating a divorced backup of the current system. Then you restore this divorced image back onto the new drive(s).

It might be possible to make a dd copy of just the B drive onto a new drive and then expanded the MFS space to use the remainer of the space, but I've never tried that.

Right now my A is a thin profile 40 and the B is a 60. Prices are dropping on drives again and I'd like to replace the B with a 120 when there's a deal at <#0.50/G again. I'm just thinking ahead.

Maybe it will be easier to just start from scratch again given I know how to do that. Better safe than sorry and it's not that difficult.
