What version of the DTivo Series 2 kernel do I need to put on a new hard drive before I use killhdinitrd hack?

That depends. If you're using version 3.x you put a killhdinitrd 3.1.1c kernel on the drive.

If you want options like folders, mrv and hmo etc, you put version 4.x on the drive with a killhdinitrd 4.0.1a kernel provided the drive is less than 128GB.

If your harddrive is bigger than 128GB for the above two cases, you'll need to have a killhdinitrd 3.1.1c kernel on the drive and monte into a lba48 aware kernel. The kernel you monte into will be either from version 3.x or 4.x, depending on the final version you'd like running on the tivo.
